Instead try and consider how much money exactly each family has. Probably not enough to cover contraceptives like The Pill, or condoms. Regardless, this is still a patriarchal society in which the men decide that it is a bad idea to wear condoms (Thanks to missionaries in the 80's and 90's telling many Africans that they make your dick fall off). Keep in mind the rate of rape in African countries is waaaay higher than the American rate.

Superstition is probably incorrect here, but to be more concise, it is cultural indoctrination, and inherent ignorance driven by a patriarchal society, with the result being poverty. I think you are correct that religion is at the root of most of their suffering, and lack of modern birth control practices.
Indeed this female problem of breast disfigurement is a problem around the world, and especially here in the US. I'm like most men I think, who wonder why women are butchering their breasts with implants, lifts, relocation, etc. In conversations with all of my friends (joking aside), men don't want a women with plastic breasts.
This issue is a woman on woman issue if ever there was one, and has very little to do with men. We are not to blame for liking young firm female shapes (that's just biology), but we are not forcing the instruments of disfigurement into your hands. At some point women have to admit, that they are at some way responsible for their own behaviour and body image as it relates to one another, and the society in which they live.

This reminds me of the time I tried out for a movie in Africa. I won't be specific, but let's just say it was not a good experience.
